Job Summary

The NP/ APRN role provides primary care and treatment to patients in a University Health Sciences clinic or hospital unit.

Responsibilities

1. Assesses the physical condition of a group of patients by performing physical examinations and obtaining medical histories.

2. Formulates the medical plan and prognosis based on patient’s condition.

3. Consults with physician(s) regarding patient health care plans and treatment.

4. Orders, interprets, and evaluates diagnostic tests to identify and assess the patient’s clinical problems and health care needs.

5. Performs selected invasive procedures for therapeutic and/or diagnostic reasons within scope of validated competence.

6. Initiates and/or recommends medical drug therapies or other forms of related therapeutic treatment and procedures in accordance with approved protocols/policy.

7. Complies with medical records policies regarding the complete and accurate documentation of patient care. Initiates and maintains required records and legal documents.

8. Participates in quality improvement activities.

9. Counsels patients, families and significant others about health and illness, and promotes health maintenance.

10. Evaluates, plans, implements, and re-evaluates the care of individuals requiring emergency measures.

Work Environment and Level of Frequency that may be required:

Nearly Continuously: Office environment.

Often: Infectious disease, oils ( there is air or skin exposure to oils or other cutting fluids ), hazards ( includes a variety of physical conditions, such as proximity to moving mechanical parts, moving vehicles, electrical current, working on scaffolding and high places, or exposure to chemicals ).

Minimum Qualifications

Licensure to practice as a Nurse Practitioner in the State of Utah. Nationally certified as a Nurse Practitioner as required under State Law. Certification in ACLS / BLS / NRP / ATLS may be required by your department or by privileging criteria. Demonstrated clinical competence and in-depth knowledge of specialty area; and demonstrated leadership, teaching, research and communication skills are required.

Utah State prescriptive privileges preferred.

Licensure is obtained by completing a Master’s degree in Nursing or equivalency (the only equivalency to the Master’s degree is current licensure as a Nurse Practitioner) and a National Certification which requires the successful completion of a nationally accredited examination. UHPPM 1-56 requires that newly hired non-physician providers apply for credentialing and privileging at the time of hire.

This position is patient-sensitive and must fulfill all associated requirements. Minimum Qualifications

Licensure to practice as an Advance Practice Nurse ( APRN ) or Physician Assistant (PA) in the State of Utah. Nationally certified as an APRN /PA as required under State Law. Basic Life Support Health Care Provider card or obtain one within one month of hire; and Advanced Cardiac Life Support certification required. Demonstrated clinical competence and in-depth knowledge of specialty area; and demonstrated leadership, teaching, research and communication skills are required. Utah State prescriptive privileges is required. Licensure is obtained by completing a Master’s or Doctorate degree in Nursing/Physician Assistant or and a National Certification which requires the successful completion of a nationally accredited examination. UHPPM 1-56 requires that newly hired non-physician providers apply for credentialing and privileging at the time of hire. Previous palliative care experience preferred but not required.

Responsibilities

Direct Patient Care: Perform palliative care consults in the hospital and outpatient settings; coordinate palliative care services for these patients and families. Perform routine scheduled, urgent, and emergent medical evaluations, order laboratory and diagnostic studies, prescribe medications and treatments, coordinate medical consultations, and participate in discharge planning for inpatients that have multiple active medical conditions and significant disease burden, in collaboration with the faculty attending physician. The APRN /PA works cooperatively with the medical students, physician residents, nursing students, nurse practitioner students and social work students. The APRN /PA follows patients as they are transferred throughout the hospital providing continuity of care to patients, families and medical teams.

Teaching Duties: Assist with development and presentation of palliative care educational programs to house staff, faculty, nursing and other hospital staff. Collaborates with clinical departments and clinical support services, such as nursing, in the development of educational programs. Serve as clinical preceptor for APRN /PA students who are enrolled in degree programs at approved universities/colleges.

Documentation of Medical Services: Complete the written documentation of medical services required by regulatory agencies including the Joint Commission for Accreditation of Health Care Facilities, the Centers for Medicare and Medicaid Services, DNV , and the third party insurers identified by the patients. Direct completion of POLST forms and advance directives.
