Job Description:

The Homecare RN Case Manager serves as a key member of the interdisciplinary team and actively manages and directs resource utilization to achieve the highest quality outcomes during a patient care experience. The incumbent oversees the effective coordination of services and manages issues in the following main areas: admission and discharge, team conference and interdisciplinary plan of care communication, patient and family education/relations, payor relations, and coordination of care for all disciplines and total fiscal management. The incumbent performs ongoing utilization and quality reviews and acts as a liaison between the payor and patient while assuring that cost-effective treatment is provided by the team. The incumbent performs RN Shared Leadership duties by supporting the fundamentals of safety, quality, experience, and stewardship by providing clinical expertise and leadership to their department. The Homecare RN Case Manager has significant leadership responsibilities (e.g., leading change, scheduling, maintenance of staff competencies, new employee selection/orientation, constructive discipline, shared leadership council representation, caregiver check-ins). The incumbent has accountability for coordination of a cohort of caregivers and actively participates in the overall operations of the department, development of department practice, and provision of patient care with specified accountabilities for implementation and outcomes.

Department: Home Health - Salt Lake Area

Shift Details: Full time- exempt, Monday-Friday business hours (Rotating weekend and on-call)

The Homecare RN Case Manager provides case management to a defined patient population in accordance with Intermountain Healthcare's policies, procedures, guidelines, and professional licensure.

Job Essentials

Understands, practices, and promotes the philosophy and guiding principles of Case Management. Develops relationships and collaborates with physicians, facilities, and teams to promote process integration, seamless transitions from one program to another, continuity of care, and to avoid duplicative care management services and processes.

Assessment/Evaluation: Manages the care for clinically complex patients admitted to homecare. Meets with the patient or family in a timely manner and conducts an initial assessment using the appropriate federal assessment.

Care Planning: Develops a patient-centered plan of care, involving the patient or family or caregiver or significant others in the process. Reviews plan of care with physician to develop goals and desired outcomes.

Advocates on behalf of the patient by communicating and collaborating with healthcare providers, payers, physicians, and community-based services, where appropriate, to establish an appropriate and integrated care plan for each patient.

Facilitates transitions of care from one healthcare setting to another. Actively participates in system and regional process improvement initiatives to improve transitions of care.

Evaluates patient response to interventions through review of goal achievement, clinical outcomes, and patient and family satisfaction.

Re-assessment/Re-evaluation: Evaluates the effectiveness of the patient's plan of care and outcomes, and modifies the plan of care or specific interventions, as appropriate.

Functions as the team leader to ensure effective day-to-day operations and problem solving, for the Home Health or Hospice team. Promptly escalates concerns to appropriate chain of command. Oversees care team and coordinates patient activities to ensure quality and cost-effective care.

Effectively and efficiently leads interdisciplinary case conferences, using collaborative practice models that promote interdisciplinary care planning and teamwork.

Completes timely and accurate documentation in the medical record using knowledge of documentation standards for the department to facilitate communication with team members. Documentation is done in compliance with all clinical guidelines and billing and reimbursement standards.

Organizes and prioritizes daily work by assessing new, current, and discharging patient needs in area(s) of responsibility.

Sets priorities and coordinates and organizes patient care delivery by effectively managing time, supplies, and resources.

Projects staffing needs for team to provide optimal patient care.

Ensures cost-effective use of staff by ensuring appropriate nurse to patient ratio and utilization of staff by monitoring use of premium pay, agency, cancellations, etc.

Oversees responsibilities for competency of staff (e.g., annual skills updates, ongoing educational in-services, completion of mandatory educational requirements).

Minimum Qualifications

Bachelor's degree in Nursing (BSN) from an accredited institution. Degree will be verified.

Current RN license in state of practice.

Current Driver's License in the state of practice.

Current Hospice Certification or Oasis Certification designation must be achieved within 12 months of hire/transition into this position.

Basic Life Support (BLS)

Two years of clinical nursing experience.

One year of experience working in Home Health or Hospice setting.

  • and -

Basic computer skills and knowledge of word processing software.

  • and -

Excellent written and verbal communication skills.

  • and -

Independent worker, who is self-motivated, has a positive attitude, and can be flexible in a rapidly changing healthcare environment.

Record as a safe driver. Must meet Intermountain Driving requirements.

  • and -

Auto liability insurance with at least the state's required minimum limits.

Preferred Qualifications

Three years of experience working as a case/care manager in a home health or hospice setting.

Experience working with third party payers.

  • and -

Knowledge of home- based health services and resources.

  • and -

Auto liability coverage higher than the State's required minimum (at least $100,000/$300,000 is encouraged).

Physical Requirements:

Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, and identify equipment and supplies.

Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.

Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, and driving a vehicle.

  • and -

Expected to lift and utilize full range of movement to transfer patients. Will also bend to retrieve, lift, and carry supplies and equipment. Typically includes items of varying weights, up to and including heavy items.

  • and -

Frequently walks and assists with transporting/ambulating patients and obtaining and distributing supplies and equipment. This includes pushing/pulling portable equipment, including heavy items. Often required to navigate crowded and busy rooms (e.g., full of furniture, equipment, power cords on the floor). Need to ascend and descend stairs or uneven surface to access patients.

Expected to drive a vehicle which requires sitting, seeing and reading signs, traffic signals, and other vehicles.
