UnitedHealth Group Supervisor, Case Management in West Valley City, Utah

Supervisor, Case Management provides front-line leadership to the Case Management staff of OptumCare-Utah. This department serves members across the continuum of care (inpatient/outpatient settings). The supervisor ensures polies and procedures are being followed by the front-line staff and measure the efficacy of their outcomes. The supervisor will identify and implement business development opportunities to support growth and ensuring the program meets affordability goals. They will interact and collaborate with interdisciplinary care team; which includes a variety of healthcare professionals and clinicians.

Primary Responsibilities:

  • Develop policies, procedures, and guidelines for the Case Management program

  • Lead patient recruitment efforts and work closely with Case Management staff to guide patient centered care across the care continuum

  • Implement regulatory standards/changes

  • Provide guidance to Case Management staff to ensure program and utilization goals are met/exceeded

  • Measure employee performance and provide coaching and counseling to staff

  • Collaborate with OptumCare-Utah providers and external partners to identify patient criteria who are suitable for case management

  • Facilitate cost effective outcomes of the Case Management program

Required Qualifications:

  • BSN and current, unrestricted RN license or unrestricted Master of Social Work (LMSW, LCSW) license in the state of Utah

  • 2+ years’ experience leading a multi-disciplinary team within an acute/post-acute setting

  • CMSA standards of practice knowledge

  • CCM certification or required within first year of employment

  • Access to reliable transportation that will enable you to travel to client and / or patient sites within a designated area

Preferred Qualification :

  • Master’s degree in related area

  • Experience in home health, hospice and/or community-based settings

  • A background in managed care and utilization review
